I had an appointment with my pulmonologist today and when she reviewed my lab work and all I ahave now been diagnosed with Cushing Syndrome. I have been having trouble with steriod induced Diabetes the last few times being on prednisone the last three time in the hospital this past year. I have notice may face was looking huge like a full moon. I have been experiencing headaches and some dizziness but thought that was part of my migraines. My ankles were getting really puffy and my back was starting to hurt as well. I have been on Prednisone daily since September but I have been on a high dose and tapering very slowly since 2/6/12 this last hospitilization.
The plan is to keep tapering slowly and keep a close eye on blood pressure and sugars. It is very embarassing going out in public with this moon face. I am a nurse have seen this before but when it you you take a different look at it. I have cried when looking in the mirror at myself several times in the past couple of days just knowing it is going to take time for this to go away. I could not imagine if I had to live like this the rest of mylife as some people have to with tumors.
I am eating a low cal diet and trying to exercise as much as my lungs will let let me right now. I asked my doctor but she did not answer me and was wondering if somone else could tell me if an inhaled corticorsteroid like Advair could contribute to Cushing Syndrome as well. As I am on the 500/50 2 puffs twice a day right now as well.
After reading all the information on this Syndrome it is scary and just another thing to worry about until I get of this Prednisone if I can ever get off from it. We have tried other methods of steriods and the Prednisone is the only one that has done the trick thus far.
Another update. I went for a doctors appointment on 2/6/2012 and ended up getting admitted into the hospital with status asthmaticus with a secondary diagnosis of klebsellia pneumonia. I was released on 2/16/2012. I am still doing nebulizer treatments every 3-4 hours and on a heavey dose of prednisone. This time my pulmonologist is going to keep me on the theopholyline as well as all the other asthma medication that I have been taking. These bouts are getting very scary. This time I needed a triple lumen central line and ICU most of the time I was there and I am not bouncing back very fast.
I am getting scared that I am actually going to die before they can get control of this asthma. This is three hospitalizations in less than six months. I have the portable nebulizer and the extra stash of prednisone and the action plan and all the emergency stuff in place but this is getting serious.
I has a friends son die three years ago because of his asthma not being under control and I feel I am heading down that road if I am not careful. My family is getting very concerned. I did fill out my living will as I feel it is time for it. I have an 11 and 13 year old daughters and I feel they need to know what my wishes are along with my husband at this stage.
I was told I was very close to the Ventilator again this time. I faught with them as I have been vented X 4 and that is not fun. They were able to keep me oxygenated with a venturi-mask at 10 LPM. What hard time to wean off all that O2.

As I approached the ER department I was asked "Can I help you?" Trying to tell the intake personnel I need to see a doctor I have asthma. She said "I have a patient for triage." On the radio. A nurse came out and asked me why I was their and I told her I have asthma ans I was really short of breath. She put the SP02 on my finger and said "98%. Lets have a seat and I am going to take the rest of your vitals signs before bringing you back to the emergency department." I at that time was really wheezing and coughing bad. I was having a hard time to talk, I walked past her and went to the nurses station as I do work in the emergency department as a nurse on a per diem basis. I could not believe how she was treating me. She was new to the department and did not know me or how bad of an asthmatic I was.
Immediately RT was called for neb treatment and ABG's were drawn. Chest x-ray was done of course that was normal but the ABG were off on room air PH 7.49 pC0241 p02 54 Bicarb 30.9 TC02 32.2
When RT came back with the results and hande them to the Doctor in the ER that afternoon it did not take long for him to come in the room and tell me that I was going to be staying for a few days.
At that time I was both relieved that I was going to get treatment finally and I wanted to tell the intake nurse just because an Sp02 stat is high does not mean a person is not in trouble. You need to assess the whole situation not just a number.
I feel we as asthmatics get have to take a lot of heat sometimes in the ER because we need to use them more often and at first if we walk in they think we are not that bad and can wait in reality we are running on adrenaline most of the time.
This little incident cost me 7 days in ICU and 6 days on Med/Surge. I hope this it for a long time. I feel weak and it is going to take a long time to bounce back from this one.
Although I did get a treatment I have never heard of before "Heliox" There is nothing funnier than talking to the ICU nurses and RT's and Doc's in a Mickey Mouse voice.
My biggest lesson to anyone about this whole discussion is be persistant when going to the ER if you are in trouble and they say your 02 level 98% you can wait and you know you can't DON'T!!!View Thread
Well since the winter has decided to come to the Northeast Kingdom here comes the asthma symptoms. I just got over pneumonia and now the cold air. I was on a taper of prednisone now back to 60 mg daily with a neb treatment lasting 3 hours. I am afraid this going back into pneumonia and my pulmonologist is suggesting IV steriods. I am getting scared with all the steriods that have been pumped through my body. Is there any other alternatives? I have tried everything and now the cold air. I do use a heavy scarf to cover my mouth when I go outside but I try to stay inside on the real cold days but I have to go out to go to work. Please send me any suggestions I would love to hear from anyone. My bone scan already shows osteopenia at the age of 36.View Thread
It seems every time I get a common cold my asthma gets so severe I end up in the hospital for 5 to a long as a 13 day stay. I have been on top of things and have called my doctor and started oral steriods early and it never fails I end up in the hospital. My family and employment suffer. I was windering is there anything else that I can so to prevent my asthma from being so severe? I am on a steriod inhaler along with an array of other medication for asthma. I call my pulmonologist at the first sign of a cold as I know that I can get into trouble quickly. I would like any advise possible. Is there vitamins that I might be missing I know Airborn is good but it seem to not work on me. Please HELP!! It is getting costly. Last year was a 13 dat stay 7 of those days in ICU and another stay of 5 days all in ICU and I went through a surgery that they said might help but I am now fighting a cold and is going into the lungs and by the way I am breathing and the nebulizer is running every 4 hours in hopes to keep out of inpatient I am hoping for a better 2012.View Thread
I was first diagnosed at the age of 18. I started out with a bad cold that went into an pneumonia and after 2 days in the hosipital I went into respiratory failure and needed to intubated and Life flighted to a bigger hospital. Was on a vent for 2 weeks. I thank the Lord for me still being a live. I have been on a vent 3 more times since. Everytime I get sick I fear it may be my last. That is why I am asking for advise. My orders are as follows... Albuterol neb treatment every 4 hour. Duonebs every hours and I have an order for albuterol neb if peakflow below 160 nebulizer X1 then wait 1 hour if still not above 160 can have another albuterol neb and then go to ER if not above the 160. I am on the Advair 250/50 normally but as soon as a cold comes on I am to increase to the 500/50 1 puff twice dailly. I am on Flonase twice daily. Singulair daily. Zyrtec twice daily. Prednisone 60 mg daily with start of a cold with a very slow titration that takes weeks to stop.
I am allergic to several antibiotics so that is last resort for the physcians to start. I also tried Xolair and the 3rd injection my tongue and throat swelled and I had trouble breathing. I cannot give up as I have a beautiful family but at times I feel like I am going to die when I get the asthma attacks.View Thread
I have had this happen to me on several occasions. The last last time in March of 2011 I had a chest cold and went to my PCP which she put me on Prednisone taper dose and the second night I ended up waking in the night SOB ans wheezing. I went to the ER thank goodness the respiratory therapist on that night new me well as the ER doc said my O2 sat was 96% and chest xray was fine and I had a chest cold and to cont. with my Prednisone and was going to send me home. The respiratory therapist explained my case and while the doc was going over my case I crashed in the ER end ended up having a 7 day stay in the ICU and 6 days on Med Surg floor for a total of a 13 day inpatient stay. I think the ER gets overwhelmed with cases and looks at numbers and does not listen to what patients tell them. I can relate to this as I am a nurse as well ans work in an ER and have to be a patient advocate when I am not a patient. Just because numbers and test results don't make sense at times if they would stop and listen to the patient it could save a life. If it was not for that Respiratory Therapist I would have 2 daughters that would be growing up without a mother today.View Thread
