My doctor told me about it when I had one. It is a very early miscarriage. It has nothing to do with exposure to chemicals, despite the name.
He said it's because pregnancy tests are now so sensitive, they're detecting pregnancies which in the past wouldn't have been noticed. The woman would have probably thought she had a slightly late period and not realized she was pregnant at all. Instead, she gets a positive pregnancy test early on, then miscarries--possibly before she can even get in and get seen by her doctor.
