Hello there: What your dog has is a fungal dermatitis. I spoke to a holistic herbal formulator. He recommended the following which is working - but you have to keep up with the regimen and not stop because the dog suddenly seems to do better.
He recommend I give my female german shepherd an herb in capsules called Nettle Leaf. I give her 2, twice a day. It is a great antifungal (also great for cancer). I now give her grain free dog food, and bathe her with tea tree oil soap which is also anti-fungal. You need to treat this from the inside out (Nettle leaf) and the outside in - tea tree oil soap. Sometimes they also get issues with their ears. When that happens, I squeeze in a few drops of oil of oregano (which is very intense - it burns a bit). So only use a few drops.
